Dear Parents,
I would like to take this opportunity to welcome you and your child to Our Lady Queen of Peace School and Parish. I hope this letter finds you well and gives you the information you need to make a smooth transition for your child into kindergarten this year.
I have attached (at the bottom of this page) the Daily Health Self-Screening Requirement Checklist (1). Every child must have their own form signed by a parent on the first day of school. If they do not have this form they are not allowed to enter the building. This form only needs to be signed and returned on the first day. After that, you need to review the questions at home before your child(ren) come to school. If you answer yes to any of the questions please keep your child home.
ARRIVAL and DISMISSAL: Please arrive on time for both drop off at 7:50 AM and dismissal at 2:15 PM at the aftercare door #1 - through the school yard).
CLASS SUPPLIES: The following class supplies will be needed. (Please bring the first FULL week of school- Monday, Tuesday or Wednesday):

LUNCH AND SNACK:
Children should bring a SMALL healthy snack and drink for snack time, and healthy lunch and drink for lunch time. On half days please send your child to school with a snack and drink or lunch and drink, but not both. Only drinks with straws or spouts are permitted. In addition to the (2) drinks, one for snack time and the other for lunch time, please send in a thermos for water breaks throughout the day. Also, it is very important that you stress to your child that although it is wonderful to share, sharing is not allowed at snack or lunch time.
The optional hot lunch program begins Monday, September 13. Parents/Guardians will be able to order lunch electronically. Details to follow.
TWO PLASTIC FOLDERS:
SCHOOL BAG: Your child should bring in their school bag everyday. Books, folders, pencil cases and lunch/snack should be kept in the school bag. No toys please!
OUTERWEAR and ALL POSSESSIONS: Your child’s first and last NAME must be on ALL possessions. Please place your child’s name on everything (each pencil, crayon, sweater-shirt, snack, lunch, etc).
UNIFORM: For your convenience, all uniform items are purchased at FYLNN & O’HARA, which is located at 2645 Forest Avenue, (718) 227-5499. Full uniform must be worn everyday, unless your child has a “Nut Card”. Sneakers for kindergarten must be WHITE and VELCRO.
BOOKS:. All workbooks and notebooks should be labeled with your child’s name and contacted the day you receive them and returned to school the very next day.
ABSENTEE NOTES: Absentee notes must be sent in on the first day of child’s return to school to be recorded. The absentee notes can be printed from the school website: https://adny.tads.com. Your child will be responsible for all homework missed during their absence.
THE NEW YORK STATE COMMON CORE: The New York State Prekindergarten Foundation for the Common Core represents the guidelines for what prekindergarten students should know and be able to do in order to be successful learners in Kindergarten. For specific information consult the website as follows:

HOMEWORK: This should take approximately 20-30 minutes each day depending on your child. Please SIGN all homework on the Homework Log, which you will receive every week. Weekly Homework (the Homework Log) will also be posted on the school’s website: www.olqpsi.com. , at first then only on Google Classroom (GC). Google Classroom log-ins will be given out the first week of school. Please work closely with your child, allow them to do it themselves but correct any errors that may occur.
In order to minimize class disruptions, pencil cases will be sent home during the week and/or on weekends. As part of the homework assignment, parents need to check that their child has the proper supplies for the school day.
PHONICS: I will be spending a week on each letter of the alphabet. During this time, the students will be asked to bring in one item and picture representing that letter, as part of a homework assignment. Every week please study the letter of the week & previous week’s letters for recognition, phonetic awareness and the printing of each letter. We encourage reading everyday with your child as part of the homework process.
REPORT CARDS: You will be provided with a password to login to our website: https://adny.tads.com, within the first few weeks of school. Quarterly report cards can be viewed at the above website. The Fourth Quarter Report Card will be distributed to the children on the last day of school. The “Parent-Teacher Conferences,” will take place sometime in November (TBA).
However, if you need to meet with me before the First Quarter Report Cards and any time thereafter, do not hesitate to send in a written note and an appointment will be scheduled at a mutually convenient time. I will be available to speak with you by appointment but if it is something quick please wait until all the children have been dismissed before approaching me.
PHYSICAL EDUCATION: Gym class will take place once a week. All children are expected to have WHITE VELCRO SNEAKERS (kindergarten’s uniform shoe). If a child does not have sneakers or the appropriate sneakers they will not be allowed to participate in gym class.
ART/MOVEMENT (DANCE)/PHYSICAL EDUCATION: Kindergarten children will participate in scheduled classes during school hours.
SCHOLASTIC BOOK CLUB: The Scholastic Book Club Orders are made on-line. Catalogs will be sent home periodically. For on-line purchases the code for your ordering will be LDKVY. Please send your order to your HOME!
BIRTHDAY CELEBRATIONS: Birthday celebrations will be acknowledged in class. A small party favor (pencil or eraser, items like that, etc.) is appropriate to give out to each student. Please advise before your child's birthday. Children can dress down on their birthday.
MISCELLANEOUS NOTES:
I look forward to meeting you on “Back to School Night,” Monday, September 13, on a zoom meeting. The zoom link and Google Classroom log-ins will be provided the first week of school. I am eager to work with you and your child this year. Please retain this packet for reference throughout the year.

All School Employees, Parents, Children and Essential Visitors will need to complete a daily health self-check by
answering the questions listed below and by taking their own temperatures each day prior to entering the school or being
on the school property. A parent or guardian is responsible for completing the daily screening on behalf of their
child(ren). Self-Screening:
Below are the updated self-screening questions that employees, parents, children and essential visitors are required to
answer daily. If the answers are “No” to all of the following questions, individuals may enter the school, if you answer
yes please contact the school principal. (If employees, parents, children and/or essential visitors cannot take their
temperature at home, but answer “No” to all other questions, they may report to the school to have their temperature
taken on-site).
a) If you are not yet fully vaccinated, have you been in close or proximate contact in the past 10 days with anyone
who has tested positive through a diagnostic test for COVID-19 or who has or had symptoms of COVID-19?
b) Are you waiting for the results of or have you tested positive through a diagnostic test for COVID-19 in the
past 10 days?
c) Do you have at least one of the following symptoms: cough, shortness of breath, difficulty breathing, new loss
of taste or smell?
d) Do you have at least any of the following symptoms: fever (over 100.0°F or subjective), chills, rigors (shivers),
muscle aches, headache, sore throat, nausea or vomiting, diarrhea, fatigue, congestion, runny nose?
If you answer YES to any of these questions on any given day, you or your child may not enter the school building.
Contact a medical professional for guidance and notify the school principal. For students with chronic illness,
only new symptoms or symptoms worse than baseline should be used to fulfill symptom-based exclusion criteria.
Depending upon the answers to the screening questions, you or your child may need to receive medical clearance and a
negative COVID test to return to work or school. If you or your child is sick with a common cold or other ailment not
related to COVID-19 and has an approved alternative diagnoses they should stay home until symptoms subside and you
or your child (ren) fever free without the aid of a temperature reducing drug.

SCHOOL
Parents should take their child’s temperature every day before their child leaves for school. If your child
has a fever of 100.0 or over, your child should be kept home from school. A fever is a key symptom of
COVID-19. You should ask your child if he/she has any other symptoms of COVID-19 (see below). If
your child complains of these symptoms or has a fever, or you answer yes to a question in section two,
please, keep them home, call your child’s doctor and contact the school to report your child’s absence. If
your child is sick with a common cold or other ailment your child should stay home until symptoms
subside and they are fever free. Those with symptoms must be tested for COVID-19 before returning
to school per the NYS DOH School Toolkit. We also strongly encourage everyone to get the COVID-19
vaccine if eligible, as well as the FLU vaccine. In NYC, Children 59 months (4 years old 11 months) and
under are mandated by NYC DOH to receive the Flu Shot by Dec 31st
.
SECTION 1: Symptoms
Before you or your child leaves for school each day – Consider all that apply:

People with COVID-19 have had a wide range of symptoms reported – ranging from mild symptoms to
severe illness. Symptoms may appear 2-14 days after exposure to the virus. People with these symptoms
may have COVID-19. This list does not include all possible symptoms. CDC will continue to update this
